SUNSHINE CAMP 2013

During each day camp, participants with disabilities will be engaged in fun activities, such as grooming, tacking, and riding the horses, along with horse-themed arts and crafts.

Children with disabilities between the ages of 5 and 21 can be registered for one or multiple days of the camp.

 The cost is $30 a day/per rider.

To be eligible for participation, you must complete the following items at least two weeks prior to the camp date:

1.Registration Form

2.Participant Forms and Medical History Form

3.Payment - Make checks payable to Slippery Rock University

Directions:

1. Print out and complete the bottom half of the registration form.

2. You will have to fill out a set of participant forms for your child before he or she can participate. You may print them, fill them out, and mail them to the address listed on the participant forms page.

3. You MUST HAVE a doctor fill out and sign the MEDICAL HISTORY form before you can be signed up to ride at camp.

4. To be eligible to participate, you must register and pay at least ONE WEEK prior to the camp date. Spots fill quickly, but your child will NOT be registered until we receive payment and paperwork! Calling to ask if the camp is open and sending payment later does not guarantee a spot. Keep in mind that a doctor must sign the medical form.
