When is my Health History form due at Student Health Services?

A completed Health History Form should be submitted no later than one month prior to your first day of class.  (Fall Semester = July 31 deadline; Spring Semester = December 15; Summer Semester = April 15). If you did not receive the Health History when you enrolled, the form can be downloaded at www.sru.edu/pages/7098.asp.
