Preceptor rating forms are given to preceptors by the student at the beginning of the student's clinical preceptorship experience.  The evaluation should be completed at the end of the clinical experience and should be discussed with the student.  This evaluation becomes part of the final course grade.  This evaluation form is based on the clinical skill objectives that the student must complete while with the preceptor.  These forms are also available in the MSN Forum on Blackboard under "Course Documents" in the "MSN Forms" folder.  There is a Preceptor Rating of Student Clinical Skills Form for each individual clinical course.
