APPLICATION INSTRUCTIONS

FRESHMEN STUDENTS

Please type or print in ink all portions that apply to you.  No one but the applicant should fill out the application.  No evaluation will occur if any required information has been omitted.  All applicants should list their Social Security Number and sign the application.  The application may be initiated as early as August 1, following the completion of the junior year. Falsification or misrepresentation of information on the application will lead to dismissal from the University.  Please do not apply more than one year in advance of the term you plan to attend.

Secondary School Transcript:  It is the applicant's responsibility to arrange to have an official secondary school transcript and test scores forwarded to the Office of Admissions at Slippery Rock University.  The secondary school should include your senior year course schedule and class rank at the close of the junior year.  A final, official transcript will be required upon graduation.

Required Freshman Testing:  All candidates for admission should take either the SAT or ACT test for application consideration. To request that scores be sent directly from the testing service to Slippery Rock University, use code 3716 for the ACT test, and code 2658 for the SAT test.  Scores will also be official if included on high school transcripts.  Applicants out of high school for more than two years need not submit test scores unless requested to do so by the Director of Admissions.

G.E.D. Note: Students who have obtained a G.E.D. should provide a copy of the G.E.D. score report and diploma, and an official transcript from all secondary schools attended until the equivalency diploma was earned.

Transfer Students: transfer students are those who have graduated from high school and have subsequently enrolled in post-secondary coursework or attended the military.  Transfer students are expected to have achieved a satisfactory grade point average at previous institutions.  Students who for any reason are on academic suspension from another institution will not be given consideration for admission for at least one calendar year after the last semester's attendance. Those who previously attended SRU as degree-seeking students, left for any reason and subsequently intend to return to the University, should not complete this application, but instead should apply for Readmission through their dean's office.  An offer of admission to the University does not necessarily imply an acceptance to the student's chosen major.

APPLICATION PROCEDURE:

1.  Application of readmission and the application fee.

2.  Official transcripts from each post-secondary institution attended (Failure to submit all transcripts will result in dismissal from the University).

3.  Official score reports (AP, CLEP) and military transcripts, if applicable.

4.  High School Transcript (for students who have attempted less than 60 credits).

Offered students are expected to submit all final transcripts.  Failure to do so will result in a hold of registration privileges.

Transfer of Credits: An official evaluation of transfer credits will be provided to the student who has been offered admission and has paid the Enrollment Deposit.  A preliminary unofficial evaluation of credits may be provided prior to application for admission, upon a student's request.  Credits are considered for transfer based on the following criteria:

1.  College level courses with grades of A, B or C are normally accepted in transfer.  Letter grades of D may be accepted from Pennsylvania community colleges and other Pennsylvania State System of Higher Educat9ion universities in some circumstances.

2.  Courses with grades of "P" or "S" are normally accepted.  A maximum of 12 credits earned on a Pass/No credit system may be transferred.

3.  Courses earned through "credit by examination" are normally accepted in transfer.

4.  A maximum of 67 credits may be transferred;

    1).  If a student attends a two-year non-baccalaureate degree granting institution, or

    2).  If a student last attends a two-year non-baccalaureate degree granting institution.

5.  A maximum of 60 credits may be transferred; 1).  If a student attends a proprietary/technical institution, or 2).  If a student last attends a proprietary/technical institution.

6.  Students must earn 48 credits of Liberal Studies coursework and 48 credits of upper level coursework to satisfy graduation requirements.

7.  Students must satisfy the Residency Requirement by earning their last 36 credits prior to graduation from Slippery Rock University.

Please return your completed application and non-refundable $30.00 application fee (made out to Slippery Rock University) to:
