DROP/ADD CARD - used to drop or add courses to one's
schedule up to and during the first week of classes in any
semester. Advisor's signatures are needed to drop or add open
sections of a class. Closed sections can only be added with the
instructor's signature and the advisor's signature.
WITHDRAWAL CARD
- used when students wish to drop a
course after the first week of the semester. A "W" can be
granted up until the end of the tenth week of the semester.
After the tenth week deadline, students may not withdraw from
courses and must be graded. Both advisor's and professor's
signatures are needed. Basic competency courses cannot be
dropped.
AUTHORIZATION OF GRADE
DISCLOSURE form -
Student's signature on this form releases a student's grades for
one academic year to their parent(s) or other persons, i.e.
guardian.
CHANGE OF ADDRESS AND PHONE
NUMBER - updates the
student's new local and/or permanent address and/or phone number as
well as their emergency phone number. The Name/Gender Change
form and policy statement permits the student to change their name
or correct gender on official University documents as specified
under the statement's conditions. These include submission of
an official document or notarized statement verifying the
appropriate changes. See form for specifics.
GRADE OPTION CARD
- signed during registration - allows
students the opportunity to select pass/no credit or regular grades
in their course work. After the second week of the semester,
the grade selection cannot be changed. Pass/no credit courses
may not be used to satisfy major requirements or liberal studies
requirements.
TRANSCRIPT REQUEST
- used to request an official copy of
your transcript. A fee of $3.00 for each transcript is
charged. To fax a transcript, the fee is $5.00. One
complimentary copy is sent to students upon graduation.
UNDERGRADUATE TRANSIENT
CLEARANCE - used to
request permission to enroll in courses at another institution and
to have the credits (not grades) transferred back to SRU.
Transient clearance status must be granted before
enrolling in classes by completing the form and obtaining the
necessary signatures and course information.
CREDIT BY
EXAMINATION - used
to give students permission to take an examination to receive
credit in a course. A fee of $50,00 per credit is charged for
the examination. The student has one chance to earn credit by
examination in that course. Permission must be obtained by
both the chairperson of the department and the test
examiner.
REQUEST FOR CHANGE OF FIRST
MAJOR OR SPECIALIZATION -
authorizes the student's request to change their current major or
area of specialization. The form needs the signatures of the
chairperson of the new major of specialization and the
student.
MINOR DECLARATION
- used to declare or add a minor
program to your degree.
UNDERGRADUATE COURSE
REQUIREMENT WAIVER/SUBSTITUTION - used to waive or substitute one course, program, or
requirement for another. It is also used for changing to
Liberal Studies from General Education if you initially entered the
University as a General Education student.
REQUEST FOR ADDITION/DELETION
OF SECOND MAJOR OR SPECIALIZATION - Recognizes the student's request to add or delete their
current second major or area of specialization. The form
needs the signatures of the chairperson of the new major or area of
specialization and the student.
VISITING STUDENT
STATUS - used to apply for
status as a visiting student at another SSHE university. The
purpose is to have your credits and grades transfer back to
Slippery Rock. Signatures of the student's advisor, academic
dean, and the Director of Academic Records all need to be obtained
before taking these classes.
REQUEST TO REGISTER FOR EXCESS
HOURS - requests
permission for a student to register for excess credit hours based
upon their cumulative grade point average. If the number of
hours exceeds 18, an additional fee is charged.
UNDERGRADUATE PERMISSION TO
ENROLL IN 500-600 LEVEL COURSES - may be completed by students completing the criteria
specified on the form. This allows them to enroll in upper
division courses and they may elect to receive undergraduate or
graduate credit as specified, upon receiving the appropriate
signatures.
GRADUATION AND DIPLOMA
APPLICATION - must
be completed by the end of the third week of the semester in which
the student plans to graduate. Students who meet all
graduation requirements but fail to complete this application or
fail to pay the necessary fees, will graduate in the semester the
application requirements are fulfilled.
APPLICATION FOR WITHDRAWAL FROM
THE UNIVERSITY - is
completed by students intending to withdraw from the University,
either on a temporary or a permanent basis. Those withdrawing
for medical reasons need to provide written documentation of their
situation. Students who withdraw and are readmitted must meet
the graduation requirements in effect at the time of their
readmission.
DEGREE AUDIT REPORTING SYSTEM
(DARs) - provides the
student and their FYRST advisor with current information on the
student's progress toward completion of their degree/major/liberal
studies requirements. DARs is distributed once a semester and
will be available for pick-up in your major department around the
fourth or fifth week of the semester. A student may also
request a copy of their DARs anytime from the Academic Advisement
in 009 Bailey Library or from the Office of Academic Records and
Summer School in Old Main. DARs reports are also available on
RockTalk online.
