Instructions to Employees in Case of Work-Related Injury
Policy #F&P 2003-20

I. PURPOSE

To describe the steps to follow in reporting work-related accidents and or injuries.

II. OBJECTIVE

1. To ensure that prompt medical attention is provided in the case of a work- related injury.
2. To provide an accurate recording and tracking system to analyze and determine the cause of the accident and to supply proper documentation to the University's third party insurance administrator (TPA).

III. POLICY

1. All work-related injuries, regardless how minor, must be reported to your supervisor and to the safety office.
2. If medical treatment is necessary, the university's panel of physicians* must be utilized for the first 90 days of treatment to ensure that treatment will be paid for by the TPA.
3. If the injury is determined to be work-related all reasonable, necessary, and related medical and surgical services, orthopedic appliances and prostheses, and training in their use will be paid for.
4. If you do not treat with one of the panel providers, your medical bills will not be paid during the first 90 days of treatment.
5. After the 90-day period, should you still require treatment, you may choose to go to another licensed physician. You must notify the University's Human Resource Office in writing of this action within five (5) days of your first visit to a non-designated health provider. Failure to do so may relieve the University of liability for services rendered prior to appropriate notice, if such services are determined to be unreasonable or unnecessary.
6. Should one of the physicians listed above refer you to another specialist, the TPA will pay the bill for the services provided.
7. Should you be faced with a medical emergency, you may secure assistance from a hospital or physician of your choice.

ALL MEDICAL BILLS PERTAINING TO A WORK-RELATED INJURY SHOULD BE SUBMITTED TO THE UNIVERSITY'S THIRD PARTY ADMINISTATOR (TPA) WHICH IS AS FOLLOWS:

8. According to the Workers' Compensation Act, prescription drugs are to be reimbursed at 110% of average wholesale price. You must pay for the prescription and submit the bill to the TPA.
