Required Forms for Taking 500 Level Courses in the School Nurse Program
Beginning Spring 2008 all school nurse courses are 500 level courses. Because a 500 level course can be taken as either an undergraduate or graduate course, this change will offer several options for students in the program. This document will explain these options and the forms needed to exercise these options. All forms can be acceded from on-line from SRU’s Academic Records Web site at http://www.sru.edu/pages/768.asp.
- If you are an undergraduate in the RN to BSN or the RN to MSN Program you will need to complete the Undergraduate Permission To Enroll In 500 Level Courses form. With this option you will earn undergraduate credits.
- If you are a senior in the RN to BSN or the RN to MSN Program you have the option of taking 500 level courses for graduate or undergraduate credit. If you would like to take a 500 level course for graduate credit and do not need the credits to meet the 120 credits required for the baccalaureate degree you should complete the Request For Graduate Credit Registration For Senior Undergraduate Students form.
- If you are a post bac student you can take the course as a graduate or undergraduate course. A post bac is a student who has completed any baccalaureate degree. If you elect to take a 500 level course as an undergraduate course you will not need to fill out any paperwork. However if you want to earn graduate credit you will need to complete the Post-Baccalaureate Permission to Enroll in Courses for Graduate Credit form.
- Students taking 500 level courses as an undergraduate course will pay the undergraduate distance education tuition. Students taking the course as a graduate course will pay graduate distance education tuition.
Again the required forms are as follows --
Permission to Enroll in 500 Level Courses (Undergraduate)
Request for Graduate Credit Registration for Senior Undergraduate Students
Post-Baccalaureate Permission to Enroll in Courses for Graduate Credit
