Slippery Rock University
McLachlan Student Health Center

Dear Parent,

Slippery Rock University, in compliance with Senate Bill 955 passed in June 2002, requires all students wishing to reside in university owned housing to provide either proof of vaccination for meningitis or a signed waiver requesting exemption after having received information on the risks associated with meningococcal disease and the availability and effectiveness of the vaccine. The vaccine, Menactra, protects students against many serotypes of the bacteria. The vaccine immunity is a one-dose injection. For more information, see the Meningitis Fact Sheet.   The cost is $95.00/injection. Many insurance companies are now reimbursing for the vaccine. You will need to contact your individual health insurance carrier to determine if this immunization is covered by your policy.

If you choose to have your son/daughter receive the vaccine at Slippery Rock University Health Services, the vaccine is available throughout the school year, Monday-Friday, 8am-4pm.

We look forward to having your son/daughter become a part of our Slippery Rock University family. Please call 724-738-2052 if any questions.

Sincerely,

Kristina B. Chiprean
Director, Student Health Services
