Field Experience Agreements

(e.g., Practicums, Internships, Rotations, Clinicals, Field Experience Classes)

An agreement between a site and Slippery Rock University is required for every field experience completed by an SRU student. A field experience is the practical application of learned theory. Field experiences are known by a number of different names including but not limited to internships, practicums, clinicals, and rotations. 

The State System has created a series of templates for agreements. If the templates are used without any revisions, legal review is not necessary. If revisions are made to a template, or a site prefers to use their own agreement, University Legal Counsel must review and approve the document. University Legal Counsel must also review and approve any amendments or addendums. 

The process for approving an agreement, whether clinical or non-clinical, is the same. Items that need University Legal Counsel review and approval should be emailed to Samantha Kelly

The type of agreement needed will vary based on the field experience. Below is an overview of each agreement as well as a description of when it should be used. Actual templates are provided at the bottom of the page. Please note each template has areas that need to be customized prior to the agreement being sent to the site for review and/or signature. 

Addendum (can be used with Affiliation Agreement or Internship Agreement)

If SRU has an active agreement with an organization but the agreement does not cover your program, an addendum can be added to the original contract. All addendums need to be reviewed and approved by University Legal Counsel.

Affiliation Agreement/Clinical Agreement

An affiliation/clinical agreement is between the University and an organization/site with the purpose of providing a clinical experience, rotation, clerkship or internship.  Any experiential learning experience (ELA) requiring the student have professional liability insurance, must use this type of agreement. 

SRU uses a Uniform Clinical Training Affiliation Agreement based on the Uniform Clinical Training Affiliation Agreement published by the Association of American Medical Colleges (AAMC).  The agreement consists of the SRU Clinical Template.  If a site accepts the clinical template as is, with no revisions, they sign the SRU Clinical Template.  This document is a PDF and is ready for signature.  If a site has revisions, they should be noted.  Once both parties agree to the revisions, the SRU Clinical Template must be signed and submitted.

Internship Agreement

If a student is not being compensated at the site of the field experience, an "Internship Agreement" should be used. 

Paid Internship Form 

If a student is employed at the site of their field experience, a "Paid Internship Form" should be used in place of an internship agreement. An employee is subject to all work rules and receives a W-2 at the end of the year. Students receiving a stipend are not considered an employee. 

CDEV Paid Internship Form

If a student is employed at the site of their CDEV field experience, a "CDEV Paid Internship Form" should be used in place of an internship agreement. An employee is subject to all work rules and receives a W-2 at the end of the year. Students receiving a stipend are not considered an employee. 

On-Campus Field Experiences

Based on the advice of University Legal Counsel, SRU programs/departments where field experiences are being completed must send a letter to the department granting academic credit. A list of items to be be included in the letter can be found under the "Agreement Templates" accordion below. This letter replaces any documentation previously required for on-campus field experiences.

PASSHE Letters of Agreement

If a student is doing a field experience with another PASSHE school, a Letter of Agreement should be used. There are two versions of the Letter of Agreement; one includes liability insurance and the other does not. University Legal Counsel does not need to review these LOA.
