Staging - Refer Students for Assistance
The Dean of Students area offers support to students through our various areas. Below are links to forms/supports in alphabetical order that students can complete to get connected to our services.
Accommodation Request
Any enrolled student who would like to request accommodations due to a disability or diagnosis, can do so by completing an online accommodation request. An SRU student ID and an SRU email must be used when completing this form.
Care Referral
Any faculty, staff, student, parent or community member can refer a student they are concerned about who is experiencing distress that is interfering with their learning, development and success. Once submitted, a member of our Student Support care team will reach out to the student and invite them to conduct a needs assessment and provide resources and support.
Conduct Referral
Any faculty, staff, student, or community member can submit a conduct referral if they feel that a student has violated the Student Code of Conduct, a set of expectations for student behavior on our campus. It outlines the rights and responsibilities of all students and provides a fair process to evaluate and respond to allegations of misconduct.
Protection Connection Webstore
Protection Connection offers safer sex and safer drug use supplies to SRU students for free. Products can be ordered through our webstore for pickup at our office at 219 Campus Success Center.
Rock Pantry+ Intake Form on CORE
The Rock Pantry+ is our food pantry providing supplementary food and resources to students experiencing food insecurity on campus. Any active student interested in utilizing the pantry can make an appointment to shop or walk-in during posted hours. To schedule an appointment to shop, students can fill out the linked intake form above.
Title IX Referral
Title IX of the Education Amendments of 1972 (“Title IX”) prohibits any person in the United States from being discriminated against on the basis of sex in seeking access to any educational program or activity receiving federal financial assistance. This includes various forms of sexual harassment and sexual violence. The most efficient and effective way to notify the Title IX Coordinator is by using the Title IX referral link above, which is the same link used for a care referral.
Please note that the privacy of student information is incredibly important and the information we receive will not be shared with parents, faculty, staff, coaches, teammates, roommates, etc., without expressed consent from the student unless the student is unable to communicate with us do to incapacity or there is a danger to the student or the community.
Wellness Referral
Students can schedule an appointment with the Health Educator for BASICS & CASICS for alcohol and drug education, Options appointments for birth control education, or general wellness education, can be scheduled with Lizzy Kline using this Booking Link.
