Clearances

You must check to make sure you have all clearances valid for the Start Date of Placement (Unless the school/district specifies differently). All details and instructions are on the Phase II section of the Four Stages of Teacher Education. 

• Act 151: PA Child Abuse History Certification (valid for 1 year)
• Act 34: PA State Police Criminal History Record (valid for 1 year)
• Act 114: FBI Fingerprints (valid for 1 year)
  • If you have an “unofficial” copy, email Devonne your UEID# and she will send you an official letter to accompany the unofficial copy.
• Act 126: Mandatory Reporter Training (valid for 5 years)
• Act 24: Arrest/Conviction Form (a new form is required for each field level)
• TB Test (valid for 2 years UNLESS school requires within the past 90 days)
